Job Description

Seeking an experienced Advanced Practice Provider (NP or PA) passionate about compassionate, high-touch wound care in a mobile, home-based setting.

  • Provide advanced wound care treatments, assessments, and patient education in patients’ homes, with a focus on personalized care and healing.
  • Develop individualized care plans, coordinate with healthcare teams, and perform procedures such as surgical debridement.
  • Work full-time with a flexible, weekday schedule—no on-call, evenings, or weekends—with local travel reimbursed.

    • Qualifications include board certification, at least one year of clinical experience, wound care knowledge a plus, and Minnesota licensure. Wound care experience and willingness to travel 75% are required.
