Job Description
The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, either a Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.
Perform comprehensive assessments and develop individualized wound care plans.
Administer advanced wound treatments, including debridement and negative pressure therapy.
Collaborate with interdisciplinary teams to ensure integrated care.
Implement prevention strategies, provide staff education, and promote best practices.
Document care activities, participate in quality improvement, and ensure regulatory compliance.
Serve as a clinical resource, advocate for residents, and engage families in wound care education.
Maintain strong communication skills, proficiency with EHR and telemedicine tools, and physical ability to respond quickly in emergencies.
