Job Description

Join a dedicated healthcare team providing exceptional post-acute care in community facilities. This Advanced Practice Provider role involves managing patient care, coordinating with staff, conducting rounds, writing orders, and maintaining accurate electronic records. Comprehensive training, shadowing, and direct oversight ensure provider support.

• Deliver high-quality patient care and communicate effectively with team members.
• Participate in patient rounds, care decisions, and scheduling diagnostic tests.
• Maintain thorough, timely documentation and handle billing-related records.
• Adhere to facility policies and provide services as required.

Ideal candidates possess strong communication, empathy, organization, and problem-solving skills. Preferred experience includes geriatrics, internal medicine, ICU, emergency medicine, or hospitalist roles. Must hold a valid Nurse Practitioner license and DEA certification.
