Job Description

We are seeking a licensed Nurse Practitioner or Physician Assistant to lead skin and wound care teams, providing expert care to improve patient outcomes in skilled nursing and home settings.


  • Lead multidisciplinary wound care teams, coordinate weekly visits, and perform bedside assessments and treatments.
  • Develop individualized treatment plans, interpret diagnostics, and track wound healing progress.
  • Collaborate virtually with medical specialists, including physicians and surgeons.
  • Travel within an assigned region, primarily in Eugene and Roseburg, with typical hours Monday–Friday, 6:00am–3:00pm.
  • Must be licensed, board-certified, dependable, and have strong communication skills. Wound care experience is preferred.
