Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an independent APRN or PA responsible for managing patient care in PALTC settings.
Their goals include improving care quality, enhancing patient and family experience, reducing preventable hospitalizations, and lowering costs.
Responsibilities encompass conducting exams, diagnosing, treating, and managing chronic conditions, providing preventive services, and collaborating with facility staff and external professionals.
They must ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives.
The role requires strong clinical decision-making, communication, and interpersonal skills, proficiency with EHR and telemedicine tools, and the ability to work independently.
Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and board certification.
