Job Description

We are seeking a full-time Nurse Practitioner or Physician Assistant to join our Wound Care/Home Health team in Centralia, WA.

  • Provide wound assessments, management, dressing changes, and debridements (training provided).
  • Participate in weekly bedside wound rounds with treatment nurses in skilled nursing facilities.
  • Conduct Home Health visits as needed.
  • Establish and implement plans of care within facility protocols.

    Ideal candidates are outgoing, energetic, and experienced in skilled nursing environments.

    Requirements include at least 2 years of wound care and skilled nursing experience, strong communication skills, current WA medical license, and wound care certification preferred.

    Benefits include health/dental insurance, flexible schedule, and paid time off. Pay ranges from $125,000 to $180,000 annually. Schedule is day shift, Monday to Friday, with on-call and weekend duties as needed.
