Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to perform primary care visits in Skilled Nursing and Assisted Living Facilities in Dover, NH, and nearby areas.

• Provide primary care, medication reconciliation, and annual wellness visits 3-4 days weekly.
• Examine patients, review medical records, order diagnostic tests, diagnose conditions, and propose treatments.
• Prescribe medications, maintain accurate records, and educate patients and families on health and prevention.
• Oversee patients’ overall health and promote good health practices.
