Job Description

We are seeking an experienced Nurse Practitioner to join our mobile primary care team serving SNFs, LTC, and ALFs in Minnesota.

  • Provide comprehensive primary care, manage acute and chronic conditions, and build strong relationships with residents, families, and care teams.
  • Perform assessments, order diagnostics, develop treatment plans, and prescribe medications within guidelines.
  • Collaborate with facility staff to ensure seamless care and support quality improvement initiatives.
