Job Description

We are seeking a friendly and dedicated Primary Care Physician to join a growing PACE Organization in Springfield, MA, on a full-time basis.

  • Under supervision of the Medical Director, provide primary care to frail older adults, including assessments, care planning, direct medical care, and evaluating care effectiveness.

  • Collaborate with interdisciplinary team members, make independent medical decisions, and perform necessary procedures.

  • Qualifications include Board Certification/Eligibility in Internal Medicine or Family Practice, with a preference for geriatrics certification. Bilingual in Spanish is a plus.
