Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide high-quality, patient-centered primary care in Skilled Nursing Facilities, LTC, and ALFs.

Deliver comprehensive assessments, manage acute and chronic conditions, order diagnostics, and develop treatment plans.

Prescribe medications, educate patients and families, and collaborate with care teams to ensure seamless care.

Maintain accurate documentation and support quality improvement initiatives.

Qualifications: Minnesota NP license, board certification, 1+ year experience in geriatric or post-acute settings preferred, strong communication and teamwork skills.
