Job Description

We are seeking a Licensed/Board Certified Nurse Practitioner to provide home-based wound care for patients in a multi-specialty practice.

  • Perform in-person evaluations and management of acute and chronic wounds in home, assisted living, or skilled nursing facilities, with about 80% of time dedicated to direct patient care.
  • Conduct medically necessary debridement and procedures within CMS guidelines to promote healing.
  • Follow infection prevention protocols and handle contaminated equipment appropriately.
  • Accurately document services in EMR to ensure proper billing according to CMS standards.
  • Use clinical judgment to assess, diagnose, and prescribe treatments and medications for wound-related conditions.
  • Must have a Master’s Degree, Montana RN license, Nurse Practitioner certification, and WOCN (or be willing to obtain within 90 days). A minimum of 3 years as a wound specialist and 1 year as a Nurse Practitioner in wound care is required.
