Licensed Clinical Social Worker (LCSW)

Job Description

We are hiring a licensed Clinical Social Worker (LCSW) with geriatric behavioral health experience for part-time or full-time roles in Union Grove, WI.

Provide direct psycho-social services, including evaluations, diagnosis, and treatment for nursing home residents via individual, group, and family therapies.

Collaborate with interdisciplinary teams to ensure quality care and continuity.

Manage 10-15 patient encounters daily, flexible to accommodate scheduling needs.

Ensure documentation complies with Medicare guidelines and standards of care.

Qualifications include a master’s degree, full state license, 1-2 years’ experience preferred, and strong communication skills.
