Job Description
Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluations and treatments in patients’ homes, enhancing communication, cognition, and swallowing for safety and independence.
li>Adjust therapies based on patient progress and caregiver input.
li>Document all activities in compliance with regulations and support reimbursement.
li>Educate patients and caregivers on communication and swallowing strategies.
li>Collaborate with interdisciplinary teams and participate in team meetings.
li>Maintain licensure and stay current with clinical best practices.
Qualifications include a Master’s in Speech-Language Pathology, CCC or CFY, NY license, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
