Seeking a Licensed Clinical Professional Counselor to provide therapy, assessments, and behavioral health support in long-term care facilities within the community.

As part of an interdisciplinary team, you will deliver individual, family, and group therapy, address conditions like depression, anxiety, grief, and behavioral disturbances, and collaborate with healthcare staff to optimize resident care.

Responsibilities include documentation using Geriatric Notes Tool, building relationships with facility staff, and supporting residents’ mental health needs.
