Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant for primary care in Skilled Nursing and Assisted Living Facilities in Keene, NH, and nearby areas.


  • Provide primary care visits, medication reconciliations, and annual wellness exams 3-5 days weekly.
  • Examine patients, review medical records, order diagnostic tests, diagnose, and propose treatments.
  • Prescribe medications, maintain accurate records, and educate patients and families about health conditions and prevention.
  • Oversee overall patient health, promote wellness, and stay updated professionally.
