Job Description
Seeking a Social Worker to provide emotional, social, and care planning services to seniors in a hospice setting, working Mon-Fri 8a-5p with on-call duties every 5th night/weekend, including holidays.
Provide direct social services, crisis counseling, and bereavement support to seniors and families.
Conduct assessments, develop care plans, and assist with discharge planning and resource referrals.
Advocate for clients, assist with financial resource information, and ensure documentation complies with regulations.
Require a Bachelor’s in social work (Master’s preferred), one year of relevant experience, Nebraska certification, valid driver’s license, and ability to work in various environments.
Must be able to communicate effectively, handle emergencies calmly, and meet physical requirements, including mobility and strength.
