The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently within PALTC settings to enhance patient care, outcomes, and experience.

Responsibilities include conducting exams, diagnosing and treating illnesses, developing care plans, and providing preventive services. The AP collaborates with facility staff, families, and external providers, ensuring compliance with regulations, accurate documentation, and participation in quality initiatives.

They manage health and safety protocols, educate staff and patients, and work independently with strong clinical decision-making skills. Excellent communication, technology proficiency, and physical ability are essential. Qualifications include a relevant degree, 2+ years of experience, board certification, and licensure.
