Job Description
Seeking a licensed APP to join a nephrology team, collaborating with nephrologists to provide comprehensive care across clinic, inpatient, and dialysis settings.
- Assess, diagnose, and manage kidney diseases through patient history, examinations, lab test interpretation, and develop personalized treatment plans including medications and lifestyle counseling.
- See patients in outpatient clinics for chronic kidney disease, hypertension, and related conditions.
- Evaluate and manage hospitalized patients with acute kidney injury and electrolyte issues during inpatient rounds.
- Perform weekly dialysis rounds, address complications, and coordinate care with dialysis staff.
- Educate patients and families on disease management.
- Collaborate with multidisciplinary teams to optimize outcomes.
- Maintain accurate documentation and adhere to regulatory standards.
Qualifications include current licensure in Kentucky, certification in Adult or Family Practice or NCCPA, with nephrology or dialysis experience preferred. Excellent communication and teamwork skills are essential.
