Job Description
The Wound Care Nurse Practitioner provides advanced wound management for homebound and long-term care patients across Southern California.
Must be a graduate of an accredited NP or PA program with an active California license and board certification.
Prior wound care experience or certification is required; experience in hospice, home health, palliative care, or assisted living is preferred.
Willing to travel extensively within Southern California, with strong communication skills and ability to document visits promptly.
Performs comprehensive physical exams, wound assessments, debridement, dressing application, and patient education.
Responsible for diagnosing, ordering tests, prescribing medications, and ensuring high-quality patient care.
Requires physical ability to lift 50 lbs, mobility to reposition patients, and a valid driver’s license.
Flexible schedule with part-time hours (minimum 24 hours/week), offering pay per visit or day, along with benefits.
