Job Description
The Wound Care Nurse Practitioner (NP) reports to the Director of Nursing and supervises nurses and medical assistants.
Job Summary:
Provides specialized wound care in home, nursing home, and assisted living settings, including assessments, care planning, patient education, and collaboration with healthcare teams to promote healing.
Qualifications: Master’s or Doctoral degree in Nursing, current NP license, wound care certification within 6 months, minimum 2 years of relevant experience, and ability to work independently.
Physical & Mental Requirements: Significant physical effort, ability to handle stress, adapt to diverse environments, and possess strong assessment and communication skills.
Working Conditions: Field-based with travel within the region, exposure to biohazards, requiring reliable transportation and a valid driver’s license.
Essential Functions: Conduct wound assessments, provide treatments, supervise staff, document care, coordinate with providers, and ensure compliance with policies and safety protocols.
