Job Description

Seeking a full-time Medical Social Worker with a Master’s degree in Social Work and at least one year of healthcare experience.

Responsibilities include providing psychosocial support, evaluating patient and family needs, and developing individualized care plans within a Home Health Care or hospice setting.

Candidate will assist patients and families in coping with health and emotional challenges, coordinate community resources, and ensure compliance with care plans.

The role involves preparing documentation, reporting patient progress, and collaborating with the healthcare team to promote patient dignity and optimal recovery.
