Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Farmington, ME, and nearby areas.

• Perform primary care visits, medication reconciliations, and annual wellness exams 3-4 days weekly.
• Examine patients, review medical records, order and interpret diagnostic tests, diagnose conditions, and prescribe treatments and medications.
• Educate patients and families on health conditions, prevention, and healthy practices.
• Maintain accurate documentation, promote health, and stay current professionally.
