Job Description

The LCSW-Social Worker for Case Management provides psychosocial support to patients with complex needs, collaborating with the healthcare team and community agencies to ensure safe discharge planning.

Interventions include assessment, referral, crisis intervention, and care management to help patients and families understand and cope with emotional and biopsychosocial issues impacting care.

Requires a Master’s Degree in Social Work, LCSW license, and BLS certification. Preferably with one year of supervised clinical experience in a medical setting and knowledge of counseling techniques and community resources.

This full-time role offers a $3,000 sign-on bonus, hourly pay between $40.00-$48.10, and benefits such as health, dental, vision insurance, paid time off, and life insurance. Work is in person.
