Palliative Care Nurse Practitioner

Job Description

We are seeking a compassionate Palliative Care Nurse Practitioner to provide comprehensive care for patients with serious illnesses, focusing on symptom management and quality of life.

Conduct assessments and develop individualized care plans addressing physical, emotional, and spiritual needs.

Collaborate with healthcare teams to coordinate complex patient care, including infectious disease management.

Educate patients and families on disease processes and treatment options.

Monitor patient progress and adjust care plans accordingly.

Document interactions accurately in EHR systems.

Perform Face-to-Face visits for Hospice Medical Director.

Requirements: Master’s degree in Nursing, current Nurse Practitioner license, clinical skills proficiency, strong interpersonal skills, and ability to work within a multidisciplinary team. Experience in palliative care and infectious disease management is preferred.

Location & Other Details: Full-time position based in Akron, OH, with relocation required. Benefits include health insurance, 401(k), paid time off, and more. Salary ranges from $90,000 to $110,000 annually.
