Job Description
Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluation and treatment in patients’ homes, enhancing communication, cognition, and swallowing abilities.
Conduct assessments using standardized tools and develop evidence-based, individualized care plans tailored to home environments. Adjust treatments based on patient progress and caregiver input, ensuring goals are functional and achievable. Document all activities in compliance with policies and regulations, supporting reimbursement and confidentiality. Educate patients and caregivers on communication strategies and safe swallowing techniques. Collaborate with interdisciplinary teams, participate in care planning, and recommend equipment or technology as needed. Maintain licensure, stay current with clinical best practices, and contribute to team development. Qualifications include a Master’s degree, CCC or CFY, NYS licensure, and CPR certification; experience with dysphagia and neurogenic disorders is preferred.
