Nurse Practitioner / Physician Assistant

Job Description

Full-time housecall practitioner position in central and rural Oklahoma, offering a sign-on bonus.

Provide in-home and assisted living care, including full exams, disease management, acute follow-ups, urgent encounters, and transitional care. Collaborate with ancillary services and clinical teams to deliver comprehensive care for conditions like CHF, DMII, and COPD. Must be licensed in Oklahoma with prescriptive authority; family practice or geriatric experience preferred.

Compensation is performance-based, with additional pay for wound care, new patients, and hospital follow-ups. Ideal candidates are detail-oriented, achievement-driven, autonomous, and flexible.

The role involves managing an established patient census with regular visits, working mostly remotely with clinical support available. Credentialing, insurance, and administrative support are provided. Pay starts at $55/hour.
