Job Description

We are seeking a full-time physician with an Ohio Medical License and Board certification in Internal Medicine or Family Practice to provide comprehensive care for underserved patients with complex, chronic conditions.

Our model emphasizes “care beyond medicine,” integrating multidisciplinary teams and offering services like pharmacy, labs, and urgent care under one roof.

Responsibilities include managing complex cases, developing transitional care plans, coordinating discharge processes, treating acute exacerbations, and preventing hospital readmissions. The role involves collaborating with care teams, engaging community resources, and practicing evidence-based medicine within the PCMH model.

Qualifications include BLS certification, EMR experience, and a passion for serving vulnerable populations. Preferred candidates have experience in population health management and working with medically and psycho-socially complex patients.

Benefits include a salary of $250,000-$275,000, 401(k) matching, health, life, vision, and malpractice insurance, paid time off, and a Monday–Friday schedule with no nights or weekends. The position requires in-person work.
