Job Description

We are seeking a compassionate Hospice Nurse Practitioner to provide holistic end-of-life care in Greenfield, Wisconsin.

Conduct patient assessments and develop personalized care plans.

Administer medications and treatments as prescribed.

Collaborate with interdisciplinary teams to ensure optimal outcomes.

Educate and support patients and families regarding hospice services.

Monitor patient progress and adjust care plans accordingly.

Maintain accurate medical records and participate in case management.

Advocate for patients’ needs within the healthcare system.

Requirements include a Master’s degree in Nursing focused on hospice or palliative care, current Wisconsin Nurse Practitioner license, and willingness to travel extensively.
