The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant working independently to manage patient care in long-term care settings.
Responsibilities include conducting examinations, diagnosing and treating illnesses, developing care plans, and providing preventive services.
They work autonomously within scope, collaborate with facility leadership and interdisciplinary teams, and ensure compliance with regulations.
The role involves maintaining accurate documentation, participating in quality improvement, and promoting health and safety protocols.
Key skills include strong clinical decision-making, communication, technology proficiency, and the ability to work independently in a fast-paced environment.
Qualifications require a relevant degree, at least 2 years of experience, board certification, and an active APRN or PA license.
