*What You’ll Do*

* *Round on-site* in assisted living, memory care, and independent living communities
* *Assess, treat, and manage acute and chronic wounds* with evidence-based care plans
* Deliver *comprehensive primary care* to medically complex senior patients
* Partner closely with nursing staff and facility leadership to ensure continuity of care
* Practice proactive medicine focused on *prevention, healing, and quality of life*

*What We’re Looking For*

* *Minnesota-licensed Nurse Practitioner*
