*Summary*
We are seeking a Licensed Master Social Worker to join our dedicated team at MiraSol Health Inc. In this role, you will provide essential support and care coordination to individuals facing various life challenges. Your expertise will be vital in fostering trust and helping our patients reach their care goals.
*Responsibilities*
* Provide care coordination for patients dealing with chronic conditions or end-of-life issues.
* Conduct assessments to determine clients’ needs and develop appropriate treatment plans.
* Utilize motivational interviewing techniques to encourage positive behavioral changes.
* Manage crisis situations effectively, providing immediate support and intervention as needed.
* Bill for palliative care services.
* Collaborate with healthcare professionals to ensure comprehensive care for clients
*Requirements*
* Master’s degree in Social Work (MSW) from an accredited program
* Licensed in the state of South Carolina as an LMSW
* Strong knowledge of crisis management and intervention strategies.
* Experience in social work settings, particularly with chronic illness or end-of-life care.
* Excellent communication skills and the ability to build rapport with clients.

Job Type: Full-time
Pay: $60,000.00 – $78,000.00 per year
