  • Participate in ongoing education to meet learning needs and assess patient/family readiness to learn.
  • Evaluate and revise care plans collaboratively, ensuring continuity of care through effective communication.
  • Deliver complex, patient-centered care based on standards, prioritizing safety and education.
  • Contribute to team goals, promote cost-effective care, and maintain a safe environment.
  • Assess patient data to determine priorities, anticipate discharge needs, and foster family advocacy.
  • Collaborate with patients and families in care planning, considering cultural factors and resource utilization.
  • Engage with research to enhance practice and bring innovative ideas for improvement.
  • Requirements include an RN license in North Carolina, a minimum of one year of relevant nursing experience, and graduation from a professional nursing program. This full-time position offers a variable schedule within a supportive team dedicated to community health excellence.
