The Clinical Nurse provides competent, patient-centered care in a home health setting, following professional standards and utilizing the nursing process.
Responsibilities include:
- Participating in patient and caregiver education, assessing learning needs, and ensuring effective communication.
- Evaluating care outcomes, collaborating with the interdisciplinary team, and adjusting plans as needed.
- Delivering complex care, prioritizing patient needs, and consulting resources for high-risk cases.
- Promoting professional growth, team collaboration, cost-effective practices, and a safe environment.
- Conducting comprehensive patient assessments, anticipating discharge needs, and fostering family relationships.
- Collaborating with patients and families in care planning, considering cultural factors, and prioritizing tasks.
- Engaging with current research to enhance practice and share ideas with staff.
Requirements include graduation from a professional nursing program, licensure as a Registered Nurse in North Carolina, and at least one year of experience caring for acute and chronic patients.
