This Clinical Nurse role involves providing skilled nursing care in a home health setting, aligned with professional standards and under the supervision of a Nurse Manager/Director.


  • Participate in patient and caregiver education, assessing learning needs and readiness.
  • Evaluate patient outcomes, collaborate with the interdisciplinary team, and communicate effectively for continuity of care.
  • Deliver competent care to patients with complex conditions, prioritizing safety and psychosocial needs.
  • Assist in developing professional goals, promote collaboration, and support quality improvement initiatives.
  • Conduct thorough patient assessments, anticipate discharge needs, and foster family advocacy.
  • Plan and coordinate care with families and multidisciplinary resources, considering cultural factors.
  • Engage in ongoing research and professional development to enhance practice.


    Requirements include a nursing degree, North Carolina RN licensure, BLS certification, with preferred ACLS and PALS. One year of nursing experience caring for acute or chronic patients is required. This full-time, onsite position offers day hours within Nash County.
