This full-time, telecommute RN Care Manager position (pay range $43.87–$58.13/hr) supports IEHP members by providing high-quality care management within the Enhanced Care Management Department.
Under supervision, you will lead assessments, develop individualized care plans, coordinate care, and facilitate transitions to ensure timely, effective support for members with medical needs.Responsibilities include engaging members via evidence-based communication strategies (e.g., Motivational Interviewing), collaborating with interdisciplinary teams, training team members, and promoting whole health principles.You will build relationships with members, providers, and community resources, ensuring documentation aligns with regulatory standards and contributing to quality improvement initiatives.Qualifications include an active RN license in CA, 3+ years of care management experience, proficiency in evidence-based communication, and willingness to travel up to 75%.
