Join a dedicated healthcare team as a Nurse Practitioner working Monday through Friday from 9am to 5pm in post-acute care settings within your community.
Provide high-quality patient care through coordination, communication, and clinical decision-making.
Conduct patient rounds, document visits in electronic records, and facilitate necessary tests and procedures.
Collaborate with nursing staff, physicians, and leadership to ensure optimal patient outcomes.
Maintain accurate documentation for billing and compliance purposes.
Possess excellent communication, empathy, organizational, and problem-solving skills.
Require Nurse Practitioner license, DEA certification, and relevant experience, preferably in geriatrics, internal medicine, or related fields.
