Job Description

This position involves providing comprehensive behavioral health services in a primary care community health setting under the supervision of the Clinical Director.

  • Responsibilities include conducting assessments, developing and reviewing service plans, and delivering individual, group, and family therapy using evidence-based practices.
  • Documentation, discharge planning, and case management are also key duties.
  • Services are offered in-person or via telehealth to a diverse client population across the lifespan.

    • Qualifications include a Master’s degree in a mental health discipline, current licensure (LPC, LCSW, LMFT), and certified with DSCHC requirements. Bilingual in Spanish and willingness to travel (25%) are preferred.
