We are seeking an experienced Licensed Clinical Social Worker (preferably LCSW) for a full-time position providing compassionate support in hospice care across multiple counties.

  • Responsibilities include working independently and collaboratively with a team to assist patients and families in their homes, assisted living, and nursing facilities, often during evening hours.
  • The role involves travel within designated communities, requiring a valid driver’s license.
  • Physical demands are typical of an office environment but also include working from various non-office locations.
