Join a compassionate hospice team as a Social Worker supporting patients and families in Montrose County. This role offers flexible scheduling, a mobile office environment, and the chance to work directly in patients’ homes within rural communities.

You’ll collaborate with an interdisciplinary team—including nurses, chaplains, and physicians—to provide education, resources, and advocacy that honor each patient’s goals and wishes.

Ideal candidates have a BSW (MSW preferred), at least one year of healthcare experience, and strong communication skills. Bilingual and elderly care experience are a plus.
