Job Description

Support patients and families in Montrose County as a Hospice Social Worker, offering flexible scheduling and a mobile office environment.

Be part of an interdisciplinary team including social workers, nurses, chaplains, and physicians to provide compassionate, personalized care in patients’ homes and community settings.

Provide education, resources, and advocacy to help patients achieve their care goals, especially in rural areas.

Ideal candidates have a BSW or MSW, at least one year of healthcare experience, and excellent communication skills. Bilingual and veteran applicants are encouraged to apply.
