Job Description
We are seeking an MD/DO contractor to provide chronic care management services across multiple states.
Establish and maintain patient relationships, conducting approximately 300-375 onsite clinical visits per month. Accurately document encounters in our EHR platform and provide daily progress reports. Available for 8-12 days monthly, with reliable transportation to travel within designated regions. Require active, unrestricted medical licenses in multiple states, strong clinical judgment, and familiarity with healthcare technology. Commitment to patient-centered care and improving health outcomes is essential. The compensation ranges from $10,000 to $15,000 per month, depending on experience and location.
