Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a skilled clinical professional, either a Nurse Practitioner (NP) or Physician Assistant (PA), specializing in wound assessment, treatment, and management in a PALTC setting.

They aim to reduce complications, improve care quality, enhance resident and family experience, prevent hospitalizations, and lower costs.

Key duties include comprehensive resident assessments, developing individualized wound care plans, performing advanced wound treatments, collaborating with interdisciplinary teams, implementing prevention protocols, educating staff, documenting care, participating in quality improvement, and ensuring regulatory compliance.

The role involves resident advocacy, family engagement, infection management, and effective communication. Candidates should have at least 2 years of experience, relevant certifications, and proficiency with EHR systems and telemedicine. Physical stamina and strong interpersonal skills are essential.
