Seeking licensed Clinical Social Workers to join a multidisciplinary team providing comprehensive behavioral health services for the geriatric population across Brooklyn and Queens.

Responsibilities include conducting assessments, developing individualized care plans, facilitating referrals, and documenting patient progress. The role involves working in diverse settings such as homes, clinics, hospitals, and via telemedicine, with a focus on managing social determinants of health like housing, food security, and transportation.

Ideal candidates have at least 2 years of experience, NYS LCSW licensure, strong communication skills, and a compassionate, patient-centered approach. Must have reliable transportation and be comfortable with a hybrid schedule involving travel within NYC boroughs.

Compensation ranges from $80,000 to $92,000 annually. This full-time position emphasizes interdisciplinary collaboration and community impact.
