Requirements

✅ Qualifications

• MD or DO from an accredited medical school

• Board Certified / Board Eligible (FM, IM, or Geriatrics preferred)

• Active or eligible state medical license

• Active DEA license

• Medicare & Medicaid provider enrollment

• Strong communication and team-based care mindset
