Job Description
Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Keene, NH, and surrounding areas.
Perform primary care visits, medication reconciliations, and annual wellness exams 3-5 days a week.
Examine patients, review medical records, order diagnostic tests, diagnose conditions, and develop treatment plans.
Prescribe medications, educate patients and families, and promote health practices.
Maintain accurate documentation and oversee overall patient health.
