Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Keene, NH, and surrounding areas.

• Perform primary care visits, medication reconciliations, and annual wellness exams 3-5 days a week.
• Examine patients, review medical records, order diagnostic tests, diagnose conditions, and prescribe treatments and medications.
• Educate patients and families on health conditions, prevention, and health practices.
• Maintain accurate records, promote health, and stay updated professionally.
