Job Description

We are seeking a licensed LCSW with 1-2 years’ experience in geriatric behavioral health for a part-time or full-time role supporting long-term care facilities in Fond Du Lac, WI.

  • Provide direct psycho-social services, including evaluations, diagnosis, and treatment of nursing home residents through individual, group, and family therapies.
  • Collaborate with an interdisciplinary team to ensure quality care and continuity of services.
  • Manage 10-15 patient contacts daily with flexibility to balance workload.
  • Utilize electronic medical records and adhere to Medicare guidelines for documentation.
  • Enjoy a flexible, daytime schedule with clinical support, autonomy, and a dedicated practice manager.

    • Qualifications include an active state license, excellent communication skills, proficiency with technology, and preferably experience in healthcare settings.
