Job Description

We are hiring a Licensed Clinical Social Worker (LCSW) specializing in geriatric behavioral health for part-time or full-time roles in Fond Du Lac, WI.

Provide direct psycho-social services, including evaluations, diagnosis, and treatment of nursing home residents.

Conduct individual, group, and family therapy with 10-15 patients daily, promoting work-life balance.

Collaborate with interdisciplinary teams to ensure quality care and continuity.

Maintain knowledge of Medicare guidelines and documentation standards.

Qualifications include a master’s degree, full state licensure, 1-2 years’ experience, and proficiency with technology.
