The Certified Family Nurse Practitioner provides compassionate, patient-centered primary care services under supervision, focusing on wellness exams, acute illness, and disease management.

Assess, diagnose, and manage patients across the lifespan, emphasizing prevention and health education.

Collaborate with a multidisciplinary team to coordinate holistic care.

Maintain accurate documentation in the EHR and participate in quality improvement initiatives.

Support health promotion efforts in underserved communities.

The role requires at least 1 year of experience in HIV care, primary care, or community health, and a valid Certified Family Nurse Practitioner license.
