Job Description
Seeking a Social Worker for a full-time role working Monday-Friday 8a-5p with rotating on-call, weekends, and holidays.
Provides direct social services to seniors and families, addressing emotional, social, and financial needs.
Conducts assessments, develops care plans, and assists with discharge planning and problem resolution.
Acts as a liaison and advocate, offering crisis, bereavement counseling, and resource referrals.
Requires a Bachelor’s degree in social work, with one year of relevant experience, and Nebraska certification.
Must have strong communication skills, valid driver’s license, and ability to work in various care settings.
Performs documentation per regulations, works in a facility and community environment, and may require travel.
Ideal candidates are compassionate, organized, and able to handle emotional situations, with physical ability to perform job duties.
